*If this is a home euthanasia request, please also include whether you plan to bury, are interested in cremation, or aquamation (private, where you have your pet's remains returned to you, or communal?)

****I am unable to transport pets over 25 pounds for cremation. I recommend 
contacting Burroughs Funeral Home (Grandstrandpetcremation.com), Good 
Shepherd Pet Services (goodshepherdpet.com ), or Cohen Pet Services 
(www.cohenpetafterlifeservices.com) for pick-up and cremation/aquamation. Alternatively, you may transport your pet yourself to a facility of your choosing.***
